    Organization Information

    • Case Assigned To: Shows the active user assigned to the case
    • Case Branch: Shows the active branch assigned to the case
    • Case Identifier: Shows the active case identifier assigned to the case
    • Organization: Shows the active organization assigned to case
    • Secondary Arranger: Shows the active secondary user assigned to the case

    Case Basic

    • Case Assigned To: Shows the active user assigned to the case
    • Case Branch: Shows the active branch assigned to the case
    • Case Identifier: Shows the active case identifier assigned to the case
    • Case Created At: Shows the date the case was created
    • Case Types: Shows the active case type assigned to the case
    • Decedent Full Name: Shows the entire name for the decedent (i.e. Title, First Name, Middle Name, Last Name, and Suffix)
    • Case Origin: Shows the source of the case (i.e. Self-Started, FDLIC, Import, etc.)
    • Case Status: Shows the active case status for the case
    • Case Status Change Date/Time: Shows the date and time the case status was last changed
    • Case Status Changed By: Shows the user that changed the case status
    • Note: Shows the note contents (creates one column per note entry)
    • Note Entered Date: Shows the date a note was entered (creates one column per note entry)
    • Note Entered by: Shows the user that entered a note (creates one column per note entry)
    • Note Origination: Shows what section in Passare a note originated from (creates one column per note entry)
    • All Notes: Shows all the notes found within a case, including the date, user, and origination
    • Case Notes: Shows all note information found within the Case section
    • First Call Notes: Shows all note information found within the First Call section
    • Vital Stats Notes: Shows all note information found within the Vital Statistics section
    • Family & Friends Notes: Shows all note information found within the Family & Friends section

      deleted or not

    Case Disposition

    • Decedent Date of Death: Shows the decedent’s date of death in MM/DD/YYYY format
    • Decedent Full Name: Shows the entire name for the decedent (Title, First Name, Middle Name, Last Name, and Suffix)
    • Case Disposition Type: Shows the active disposition type assigned to a case
    • Client Service Type: Shows the active client service type assigned to case
    • Body Present: Shows “true” if Body Present drop-down is marked as “Yes” and “false” it is marked as “No”
    • Final Disposition: Shows the disposition location for a case
    • Disposition Date: Shows the date of disposition in MM/DD/YYYY format
    • Case Identifier: Shows the active case identifier assigned to the case
    • Disposition Time: Shows the time of disposition in standard time format with AM/PM designations (10:00 am)

    Decedent Basic

    • Decedent Date of Death: Shows the decedent’s date of death in MM/DD/YYYY format
    • Decedent Full Death Address: Shows the End of Life address information (i.e. Address Line 1, Address Line 2, Address Line 3, Address Line 4, City, State, Zip, and Country)
    • Decedent First Name: Only shows the first name of the decedent
    • Decedent Full Birth Address: Shows the Beginning of Life address information (i.e. Address Line 1, Address Line 2, Address Line 3, Address Line 4, City, State, Zip, and Country)
    • Decedent Gender: Shows “Male”, “Female”, or “N/A” if nothing is selected in the Gender drop-down in the Vital Statistics section
    • Decedent Last Name: Only show the decedent’s last name
    • Decedent Marital Status: Shows the selection in the Marital Status drop-down in the Vital Statistics section (i.e. Never Married, Married, Civil Union, Widowed, Divorced, and Unknown)
    • Decedent Middle Name: Only shows the decedent’s middle name
    • Decedent Suffix: Only shows the decedent’s suffix
    • Decedent Title: Only shows the decedent’s title
    • Decedent Age (in Years): Shows the decedent’s age in numeric format (i.e. 15, 25, 30, etc.)
    • Decedent Age (as Text Description): Shows the decedent’s age in numeric format, followed by the unit of measurement (i.e. 50 years, 11 months, etc.)
    • Decedent Birth Date: Shows the decedent’s date of birth in MM/DD/YYYY format

    Contract Basic

    • Has Signed Contract: Shows the date (MM/DD/YYYY) and time (military time) the contract was signed
    • Has Unsigned Changes on Contract: Shows “true” if there are unsigned changes to the contract within the report’s date range and “false” if they are not any
    • Payment Due By: Shows the payment due date for a contract in MM/DD/YYYY format
    • Contract Date - First Signing: Shows the date of the contract’s first signing in MM/DD/YYYY format
    • Contract Date - Last Signing: Shows the date of the contract’s last signing in MM/DD/YYYY format

    Custom Fields

    • Custom Field (1-20): Shows the Custom Field name in the column header and the entries made to that custom field within the case

    DECEDENT INFORMATION
    Decedent Basic

    • Decedent Date of Death: Shows the decedent’s date of death in MM/DD/YYYY format
    • Decedent Full Death Address: Shows the End of Life address information (i.e. Address Line 1, Address Line 2, Address Line 3, Address Line 4, City, State, Zip, and Country)
    • Decedent First Name: Only shows the first name of the decedent
    • Decedent Full Birth Address: Shows the Beginning of Life address information (i.e. Address Line 1, Address Line 2, Address Line 3, Address Line 4, City, State, Zip, and Country)

    • Decedent Citizenship: Shows the Citizenship information found within the Vital Statistics section
    • Decedent Ethnicity: Shows the Ethnicity drop-down selection within the Vital Statistics section (i.e. Hispanic: Other, Hispanic: Cuban, etc.)
    • Decedent Full Address: Shows the decedent’s full address information (i.e. Address Line 1, Address Line 2, Address Line 3, Address Line 4, City, State, Zip, and Country)
    • Decedent Race: Shows the Race drop-down selection within the Vital Statistics section (i.e. African American, Chinese, White, etc.)
    • Decedent Religion: Shows the Religious Preference drop-down selection within the Arrangement section (i.e. Agnosticism, Christianity, etc.)
    • Decedent Social Security Number: Shows the decedent’s full social security number, including dashes (000-00-0000)
    • Decedent Tribe: Shows the decedent’s tribe information within the Vital Statistics section
    • Notes: Shows notes from the Vital Statistics section
    • Veteran Date Enlisted: If the decedent is a veteran, it will show the date they enlisted in MM/DD/YYYY format

    Decedent Death Information

    • Decedent Date of Death: Shows the decedent’s date of death in MM/DD/YYYY format
    • Decedent Time of Death: Shows the time of death in standard time format with AM/PM designations (10:00 am)
    • Decedent Full Death Address: Shows the End of Life address information (i.e. Address Line 1, Address Line 2, Address Line 3, Address Line 4, City, State, Zip, and Country)
    • Decedent Death Address Inside City Limits: Shows “true” if the decedent’s death address is marked “Yes” for being within city limits within the Vital Statistics section and “false” if marked “No”
    • Hospital Place of Death: Shows the selection in the Place of Death drop-down in the Vital Statistics section (i.e. In-Patient, Emergency Room, etc.)
    • Location of Death - Name of Facility: Shows the Rolodex entry name for the location of death in the Vital Statistics section
    • Location of Death - Type of Facility: Shows the selection in the Location of Death drop-down in the Vital Statistics section (i.e. Hospital, Nursing Home, etc.)
    • Hospice Involved: Shows “Yes” if the Hospice Organization Involved check box is checked within the Vital Statistics section and “No” if it is not checked
    • Hospice Organization: If the Location of Death drop-down has a “Hospice” selection and Hospice Organization Involved checkbox is checked, then “Yes” shows; if not, then “No” shows
    • Decedent Address City: Shows the information found on the City drop-down in The Vital Statistics section
    • Decedent Address Country: Only shows the decedent’s city country information
    • Decedent Address Inside City Limits: Shows “true” if the decedent’s death address is marked “Yes” for being within city limits within the Vital Statistics section and “false” if marked “No”
    • Decedent Address Line 1: Shows the information found on the Address Line 1 field in The Vital Statistics section
    • Decedent Address Line 2: Shows the information found on the Address Line 2 field in The Vital Statistics section

    • Decedent Education: Shows the selection of the Level of Education drop-down within the Vital Statistics section (i.e. High-School Diploma, Master’s Degree, Doctorate, etc.)
    • Decedent Employer: Shows the information in the Employer field within the Vital Statistics section
    • Decedent Industry: Shows the information in the Industry field within the Vital Statistics section
    • Decedent Occupation: Shows the information in the Occupation field within the Vital Statistics section

    CONTRACT INFORMATION

    Contract Basic

    • Has Signed Contract: Shows the date (MM/DD/YYYY) and time (military time) the contract was signed

    • Contract Date - First Signing: Shows the date of the contract’s first signing in MM/DD/YYYY format

    Contract Financials

    • Has Signed Contract: Shows the date (MM/DD/YYYY) and time (military time) the contract was signed
    • Contract Current Balance: Shows the total current balance owed for the case
    • Contract Sub Total: Shows the total dollar amount of the contract, minus taxes
    • Contract Total Amount: Shows the total contract amount, including taxes
    • Contract Total Tax Amount: Shows the total amount of taxes for the contract
    • Contract Total Taxable Amount: Shows the total amount of the contract that is taxed
    • Contract Total Non-Taxable Amount: Shows the total amount of the contract that is not taxed
    • Beginning Balance: Shows the starting balance of the contract
    • Ending Balance: Shows the ending balance of the contract
    • Payments Delta: Shows the change in total amount of payment type transactions during the report’s date range
